Visceral leishmaniasis and renal tuberculosis in a patient on maintenance haemodialysis.

چکیده
Infection in patients receiving maintenance haemodialysis therapy remains a common cause of morbidity and mortality. Alterations in immune response induced by the uraemic state increase the susceptibility to infection and appear to play an important role in determining the type, incidence and outcome of these infectious complications. In this setting, opportunistic infection depends upon the epidemiological exposure of the patients and the state of immunosuppression. We report on a case of visceral leishmaniasis associated with renal tuberculosis in a patient living in an endemic country who had been treated with haemodialysis for 19 months. Both the opportunistic infections and their clinical courses are remarkable, and a review of previously reported cases is provided. Antimonial derivatives and antituberculous therapy induced a rapid and complete remission.

A case of visceral leishmaniasis (VL) in a 77-year-old woman, with renal failure on haemodialysis, admitted in the intensive care unit (ICU) with vascular instability requiring vassopressor treatment, is presented. Initially, no co-infection could be detected. The patient initially responded well when liposomal amphotericin B was administered, after bone marrow demonstrated multiple intra-cellu...
متن کاملCutaneous leishmaniasis: report of rare cases in Nepal.
Cutaneous leishmaniasis is rare in Nepal although visceral leishmaniasis (kala azar) is common in the Terai region. In country like ours where tuberculosis is more prevalent cutaneous leishmaniasis is very likely to be mistreated as cutaneous tuberculosis, especially lupus vulgaris. Here we report a series of four cases of cutaneous leishmaniasis who presented in the out patient department at T...

We admitted a maintenance haemodialysis patient with hypotension and weakness. He had undergone cadaveric renal transplantation 19 years earlier. He had lost his kidney due to chronic allograft nephropathy 1 year after transplantation and returned to haemodialysis. He had hypotension, especially remarkable during haemodialysis. He also had hepatitis C but no cirrhosis. The patient denied a prev...
متن کاملVisceral leishmaniasis and tuberculosis in patients with HIV co-infection.
We describe here two cases, one male and one female, both age 40 years, with visceral leishmaniasis and HIV-1 co-infection. The female patient had features of Koch's abdomen. The male patient had features of tuberculous lymphadenitis and bilateral pleural effusion more marked on the right side. Both were treated with highly active antiretroviral therapy, antituberculous drugs, antibiotics, anti...

Visceral leishmaniasis is a protozoar disease caused by L.donovani. Transmission to humans (incidental host) occurs via insect bite and the phlebotomous (female sand-fly) transmits the disease. Common presentations of the disease include hepatomegaly and splenomegaly, but fever, chills and night sweats may be part of the disease's symptoms. Lymphadenopathy without systemic manifestations i...
